Extracardiac anastomosis in coronary ostial occlusion.

چکیده

In his review of the physiology of the coronary circulation, Wiggers (1936) enumerated three types of compensatory anastomosis that may develop after coronary occlusion: (1) through development ofnew ititercoronary communications, (2) through formation of extracardiac communications, and (3) through enlargement of arterio-luminal channels. Widespread minor anastomoses are known to exist between branches of the coronary arteries and branches of extracardiac arteries around the major vessels at the root of the heart, and it is believed (Hudson, Moritz, and Wearn, 1932) that this potential extracardiac coronary collateral circulation may be of significance in compensating for sclerosis of the large trunks of the coronary arteries. It is not generally appreciated, however, that such extracardiac collateral arteries may attain a considerable calibre, as in the case reported here.

Augmentation of the Extracardiac Anastomoses of the Coronary Arteries through Pericardial Adhesions

The examination of four hearts, with partial or complete obliteration of the pericardial sac by fibrous adhesions, after injection of the coronary arteries with a colloidal suspension of lamp black showed that the extracardiac anastomoses of the coronary arteries were increased owing to the presence of adhesions. In all four instances a particularly rich injection of the parietal pericardium wa...

Successful Antegrade Recanalization of Chronic Total Aorto-ostial in Stent Occlusion of Right Coronary Artery with Previously Implanted Protruding Stent

Recanalization of coronary artery chronic total occlusions is a rapidly developing domain of percutaneous coronary interventions requiring considerable operator expertise. Tackling any aorto-ostial lesion with previously implanted stent that protrudes into aorta presents anatomical and technical challenge significantly reducing procedure success rates and is an obstacle when considering antegra...

Syphilitic coronary ostial

Two young Coloured men with proven syphilitic coronary ostial stenosis had severe angina pectoris unresponsive to conventional medication. One underwent an aortic valve replacement for severe aortic insufficiency associated with subtotal ostial occlusion of the right coronary artery (RCA), which was corrected by an aortocoronary bypass graft; the left coronary artery (LCA) ostium was normal and...

Giant left coronary ostial aneurysm after modified Bentall procedure in a Marfan patient.

We performed surgical repair of a giant left coronary ostial aneurysm after aortic root replacement using composite valve graft (modified Bentall procedure) in a patient with Marfan syndrome. Aneurysmal formation in the left main stem itself is very rare. In order to avoid mobilizing the coronary ostium from severe adhesions after previous surgery and to reduce the tension on the anastomosis, t...
